Muscle twitching

I am 45 a few days ago I started having twitches in some of my body started on my left hand with the middle finger, then the bottom of my feet got weird sensations, and now the top of my right leg I have been feeling twitches, and when I place my hand over my leg, I can feel the nerve jumping, and I can feel it when I am walking.  I have a rash on the top of my left foot that comes & goes.  I also from time to time feel short of breath.  I have been taking care of someone that is seriously ill for the last 10 years.  I rarely go anywhere with no one to help or won't help I should say.  I live with constant worry something will happen to me, and there will be no one to take care of my loved one.  I don't sleep much, because she doesn't.  I don't eat right, and I lived on Pepsi to keep me going.  I have done this for 10 years.  I started having fear of ALS, which brought me searching I knew that was one of the symptoms.  My right arm feels stiff & sore, but that is the only other thing I can think of bothering me, and I overuse it I know from lifting her around so much, she is unable to do anything for herself.  I am up it's 5:00am, scared to death cause of that twitching in my leg, so it won't let me sleep now that she is sleeping again.  Anyone know what this could be besides ALS? I always pray nothing happens to me, because she doesn't have anyone, but me.
