What type of illness can cause muscle weakness and faculations, numbness in extremities and elevated cpk.

Recently I have been experiencing several health problems and has resulted in a lot of weakness with moderate muscle pain and faculations, as well as numbness and tingling in extremities, and slight difficulty walking and swallowing.  Last year I had suffered an illness that resulted in a high fever (103.7) with a severe headache that extended all the way into my spine, nausea, vomiting, diarrehea and exteme fatigue.  After two days the fever receeded and most of the symptoms subsided but I was left with extreme fatigue that lasted around 3 weeks.  Afterwards I started to regain stength and did farely well for about a month then suffered a debilitating burning tearing headache, extreme vertigo and fatigue.  My doctor did a Head CT and spinal tap and both came back relitively normal with the exception of a slight neutophil count leucocyte in nature.  I remained weak for about two more weeks and rebounded back to normal but it didnt last and after another 3 weeks had another bout of headache followed by 1-2 weeks weakness.  This has become the norm for me now presenting with 3-4 weeks of well being then a low grade fever of 99.7-100.3 and joint aches for one or two days, then a sight headache and fever followed by 7-10 days of extreme exhaustion making it difficult to even rise from bed.  In this time I also started developing digestive problems and was diagnosed with enteroparisis in Dec 2008 and place on Reglan then Amitiza with no results.  Then, in June of this year I suffered a really bad bacteria infection that started with sinus congestion and went into my chest. Some how it got into my bladder and caused a really bad kidney infection.  Oral antibiotics cured the chest infection but not the kidney infection and I developed a really high fever and severe back pain.  I went into the hospital and received a 24 hour recephin iv and once the fever dropped was discharged.  Within 24 hours the fever again spiked to 103 and I again had to receive a treatment of recephin.  I went 3 day with no more symptoms then one day woke with a slight back pain that gradually progressed and by the next day left me with severe pain and spasming in my lower back that traveled up to my shoulder blades and  in so much pain i could not walk.  I was brought into my doctors office and in doing a back xray it was notice I had a colonic bowel obstruction and agian was hospitalized for a few more days.  After the bowel obstruction cleared I was still left with the back pain and a few days later was given a prednisone shot which made the pain stop but left me with still muscle stiffness and numbness in my legs.  About a week later I noticed my right leg jumping while resting then muscle spasms and soreness.  Soon it spread to the other leg and it became difficult to stand and walk.  My doctor sent me to a rhematologist who did a bunch of lab work and it came back with a CPK of 2048 and Aldolase of 16. I also had a vitamin D and slight B12 deficiency.  While waiting to see the neurologist the weakness has spread from my calves up into my thighs and eventually into my arms.  Now I am having numbness and tingling in both my legs and arms, weakness in my legs and hands and some lost of dexterity.  The muscle spasms have increased and spread and now can be seen and felt in both my calves, thighs, forearms and behind my back and shoulder blades.  I even occasionally have them on the bottom of my feet and cheeks of my face.  My primary put me on a few weeks of prednisone  to see if it helped and it made the pain go away but the spasms and weakness remained and the pain returned right after the prednisone finished.  I seen my rhematologist last week and she retested the CPK and Aldolase which came back normal this time but she said that could have been due to the prednisone.  I seen the neurologist today who checked my balance and reflexes.  After telling him I had some difficulty with my medication recently sticking and some difficulty choking on fine grains and rice he checked my gag reflux and said it was also very weak. ( He stuck the stick down my throat twice with no gag at all).  
He said the nerve conduction study was normal but the emg was abnormal mainly in my legs.  He will be sending me for a nerve and muscle biopsy and brain mri soon.  Does anyone have any ideas what type of illness could be causing this?
