I am just a guy who went through the cycle of Accutane. Just to tell you off the start. After 8 months of taking it I probably had 2-3 months of clean skin, then it all came back although I never went back to it since side effects might be devastating. Anyways When I took Accutane I started with 10mg...after 3-4 weeks my doc. increased the dosage to 20mg and after another month or so to 40mg. I never heard that a doc. would decrease a dosage in mg from 20 back to 10. I think the only reason that would be as if the blood check your son does or should be doing every month shows something is wrong with him. If you do the test every month, then definitely ask him if everything is alright. And as you said his acne came back then the dose should be definitely increased, but only under doctor's supervision and good blood test results.
The dose is waaaayyyyyyyy to low- don't even get on accutane unless your going to do a good dose. You will not get the results you want. Most dermatologists wont even prescribe Accutane that way.
