Accutane & Liver enzyme

I started accutane (20mg) treatment in Jan 2010 and my blood test results were excellent (Note: ALT= 15, AST=12).
after 27 days my hand was broken due to playing football, i went to hospital and they gave me drugs for bone. during that i called my dermatologist and i informed him about that, and i told him there is 3 capsules from accutane. so can i take them with bones drug ? he informed me no problem take them and make blood test when you finish all accutane course. after 4 days i did blood test and i surprised that the ALT = 84 , AST = 46.
I went to dermatologist  and he informed i should stop accutane for 1 month and then make new result.
the next result was ALT 64, AST 35.. i did 4 tests after that and the ALT results always above the range , my doctor send me to hospital to make altrasound test. the result of altrasound was good in general but they found that there is mild fat on liver.
i forget accutane for 1 year .. but the acne is very bad thing. i went to dermatologist to see if i can go again with accutane, and i did full test .. the ALT enzyme was 41 , AST =24 , Bilirubin = 1 .. my doctor said that the ALT and Bilirubin is limits and he wrote for me accutane 10 mg for 1 month and to make another test after 2 weeks from first day.  
and here i want to ask you, is accutane elevate the liver enzyme like what happened in my case ?! and why liver anzym didn't go back to original point 15 (first test) ??
moreover, is it risky thing to restart accutane with such resuls ? and with mild fatty liver ?
please your help.

note: male, 32 years old, W = 78 Kg, tall = 178.
