Hi there!

Well, without a detailed clinical evaluation, it would be difficult to determine the cause of the lesion. Possible causes include cyst, comedone, residual scar tissue, wart etc. Since this has been present for about 2 years, topical medications alone are unlikely to be effective in most cases and systemic/ interventional therapies may be required. I would suggest getting this evaluated by a primary care physician or a dermatologist for an accurate diagnosis and appropriate management.
Hope this is helpful.

Take care!
