My poors are really visible

The pores on my face, especially my t-zone area are becoming really visible. My nose gets clogged with more blackheads everyday. My mom's pores are extremely visible too.
I don't know whether this is being caused by my acne and/or acne medication. Or simply a skin type that I unfortunately inherited.
Anybody have advice on what to do to minimize them?
