Scalp Acne at 49

I'm a 49 yr old male scalp acne sufferer. I took 40mg of accutane in 1986 for 4 months to treat cystic acne. The acne relapsed in 2002 and I took a 2nd course of accutane for 6 months at 40 mg. Unfortunately, 4 mos later, the acne returned on the scalp. I took a 3rd course at 80mg for 6 mos. Now, almost 5 yrs later, the acne has returned on the scalp. I've tried prescription strength Nizoral, Selenium Sulfide, Tar Shampoos and a course of Minocycline. The scalp acne returned. Can I take another round of accutane? If so, should I increase the dose? I weigh 180lbs.
