Very embarassing situation with acne, could use advice badly

Hi again everyone, I'm really hoping I can get some help here with my situation.

I am 35 years old. I am a professional driver, but that shouldn't have anything to do with this as I've had it for years and years. I am overweight, about 270 pounds, but 6 foot 1, so it's not as noticeable as with some people. However, my problem lies with what is highly noticeable to anyone around me when I am not fully clothed and it's becoming a serious issue.

My thighs, usually around the waist area, inner thighs and sometimes outer thighs as well are covered with acne in all different sizes. Sometimes, near the waistband area, I get some that are so large and so painful it's almost intolerable. On my inner thighs, the acne is generally much smaller, but these last forever! I've had this condition since as far back as I can remember, in my adulthood anyhow. The pimples, even if popped, stay there for months sometimes, before the skin goes back to normal.

I have the exact same issue on my stomach. Close to the waistline again. Some of these are also large and painful.

My chest is, for the most part, clear of acne, as are my arms, and very few on my face, maybe one a month, if that (Have one now, hate it).

See, the problem is, I can't get rid of these. I've tried everything from changing my linens nightly (which I simply cannot keep up with), trying different laundry detergent, different soaps, pretty much anything I could think of. I am in a new relationship right now and I don't even take my shirt off due to the embarrassment I feel because of all this acne.  It is seriously having a detrimental effect on my life, and I want it gone, but it's been there for so many years I have no idea what to do about it.

If I had the occasional pimple here and there, fine, but when I get rid of one and a month later there's still a large red bump there, they're building up, and I'm at a loss.

Please, any suggestions? I can't keep going on like this. I can't afford a dermatologist right now, and working 90 hours a week doesn't leave me much time to even consider going to a normal GP ...

Oh, and I forgot to mention, I am also getting a lot of acne, similar to what is on my thighs/stomach on my upper and lower back. These can also be very large and painful. If it is of any value to mention, I've also had 2 sebaceous cysts, one on my thigh which I managed to get rid of after weeks of squeezing as hard as possible, and one I still have on the middle of my back which sticks out about the same size as a 3rd of a golf ball. (if you can imagine a golf ball embedded in my back with the top sticking out, you can see how big it is) ... I have no idea if the cysts are related to my acne or not.
