acne/ingrown hair

I'm a 36yr old male. For the past several years I've had a problem with my face breaking out. At first, I thought it was just normal acne but i think it's something more.  Everytime I get a bump (which is red and swollen) I end up using a needle to dig out an ingrown hair.  I'm not sure what to do.  If I dont get the hair out it swells up even worse then if I just get it out.  Sometimes, my face will clear up some but never fully! I've tried EVERY kind of face wash, strips, and over the counter treatment there is.  I even went to a dermatoligist who told me to quit using whitening tooth paste!  I'm at the end of my rope becuase it seems to be getting worse.  It's very embrassing since I go to work everyday.  Please give any suggestions! I've yet again made an appointment with a different dermatologist.
