week 24 of accutane with absolutely no improvements only worsening

Week 24 of accutane and still not helping. My 14 yer old son is on week 24 of accutane with no improvement only 10xs worse acne. He has been on prednisone and erythromycin also for four months. He is only on 40mgs of accutane a day because of this terrible flare. I don't know what to do, why did it make his acne so much worse and why is it not helping at all? I understood you could have initial breakout  but we are 24 weeks into this with no improvement only severe worsening. The dermatologist doesn't seem to know why this is happening. He was taken off the erythromycin yesterday and put on cipro for 10 days. I don't know what to do, he is desperate for relief from these horrible cysts the accutane is causing. Is there anything we can do? Please help.

Accutane takes some time to work. Your acne will get worse before it gets better. Accutane first brings your blemishes to the surface causing more ancne, or even cystic acne. Give it some time.

Strange though, as my pharmacy print out says that acne may worsen the first few days, and it may take 1-2 months before noticing full benefit.  Also that if severe acne returns, a second course of treatment may be started after you break from the medicine for 2 months.

Did your son break after 5 months?

40mgs isn't exactly low.  I was started out on 20mg the first month, and now the second month I'm going up to 30mg.

I've been on it close to 4 weeks.  I had some worsening in the beginning, but around the 3rd week have noticed great improvement.  

I don't understand why it isn't helping your son.  Has your son tried taking a break from all dairy?  That can be a huge problem for skin.
