i've done some more research on this drug. as far as i know, the fortwin with promethazine injection isn't available in the US. both drugs can be found in different oral preparations, though.

your mom seems to be dealing with chronic pain. as a hazard of treating chronic pain, there is always the very real risk of addiction. in a true chronic pain sufferer, addiction may be someone one just has to learn to manage rather than conquer. ultimately the best thing to do would be to work closely with someone who deals with chronic pain and have every test in the book done. find doctors who specialize in migraines.

this addiction is particularly dangerous for a few reasons one of which seems to be tissue damage at the injection site. the biggest part of this is whether your mom wants to get off this drug or not. that determines everything. second is trying preventative meds like topamax or imipramine or even herbal meds like fever few. a good doctor will consider all these if she hasn't already tried them. fever few is something i just recently found out about. i don't know how effective it is in the general population, but i do know someone who actually has used it to great success. its just something to consider with  a doctor. beta blockers like propranolol can also be effective. sometimes finding the right combination therapy is the trick.

in general, barbiturates (such as butalbital products) and opiates are last resort treatments. if all drugs fail and the doctors are out of ideas, then as a last resort would come management via barbiturates or opiates....and this is often tricky because using the wrong meds to start out with can lead to a very rapid and high tolerance and effectively making them useless. barbiturate therapy tends to be very effective for some, although for others it makes their migraines much MUCH worse and more frequent. in addition, it can also be highly addictive. opiates tend to stop working over time and are obviously highly addictive.

i don't know how much your mom has already done. there are LOTS of new treatments and therapy's for migraines so if she started going to doctors 18 years ago, the world of headaches has changed a lot since then. if she is interested in changing her current situation, she may also find the pain management or migraine forums useful. if she is addicted to this drug, then it could be causing more pain than she would otherwise have if she were not taking it at all. if she's an addict, she might benefit from using this forum herself.

thats all i've got. don't know that it even helps. maybe i'm telling you things that she already knows and has tried. i don't know. i hope she finds relief, though. chronic pain for 18 years....thats terrible.

Ppvarun,

I am sorry to hear about your mom. Unfortunately, the majority of members on this site are in the US and I, for one, am not familiar what what types of treatment are available in your country---which I assume to be India. If you have treatment facilities available, I suggest doing a search or speaking with her doctor about what is available. Also, have you spoken directly with your mom? What does she say about stopping the medication?

wow i had to look that up just to find out what it was. is she wanting to stop taking this medication? if she's addicted, she'll have to want to stop before anything you can do will be effective.

it was hard to find any information on this drug...but i'm gathering its an opiate mixed with promethazine....is that right?
