help me please!

my addiction started out using lo dose percs (5 & 10) about 15 a day. Since 1994 my addiction took me to using #16tabs of 80mg oxys and # 10tabs of 10/325 percs DAILY.  I tried to ween off myself and eventually couldn't take the w/d so i checked into rehab for 21 days.  I walked out of rehab feeling great, but that last only 4 days, until i picked up again (STUPID...I KNOW!!!!)  So here I am fighting a #9 tab of 80mg oxy and 8-10 of the 10/325.  I was too embarassed to go back into rehab rationalizing that this run was not nearly as long as the other one,  So I tried to w/d myself again.  But so terrified of the w/d, i went to see a sub dr. yesterday.  He put me on 16mg/2 of subxone film.  Though my original w/d symptoms have not completely gone away, stilll anxious, agitated restless, aching, but my real question is how long should i stay on the sub w/out having to go thru w/d all over again?  I'm so scared and want my life back.  I want freedom and be totally clean even from the subs.  Any advice will be so greatly appreciated.  Thanks,
