yep, sure did....had to sign one with my pain mgmt dr. as well as with my neurologist (whom i went to before finding the pain mgmt dr.)....had to sign it, agree to use only ONE pharmacy as well, for any meds prescribed, didn't matter if something happened during the month (e.g., i lost them, they were stolen, accidentally dropped down the drain, eaten by the dog, whatever, didn't matter....still wouldn't refill prescription until my 30 days were up)....pain mgmt contracts are common place around here....don't know of any pain mgmt doctor around here who doesn't use one

I had to sign one with my neurosurgeon as well and also have to use the same pharmacy/ Plus it has many other things in there as well.

Main thing is that it all depends on the DR office and their take on it.  It all varies.  But if you decide to not sign one when they ask you to, then get prepared not to get any meds.

I had to sign one with my nurosurgeon that said I would not get pain meds from any other dr without telling him about it.  And I had to agree to always use the same pharmacy too.

It is up to the doctors more and more of them are starting to use them . .It is a pretty smart thing do to .I am sure it takes some liability off the doctors.

i was curious, because in our town some ppl. have to sign them and some don't...in fact i have 5 other friends that are on some pretty heavy duty pain meds.  and none of them had to sign a contract and i did.  just thought it was strange....

I had to sign one - my doctor's office started because of the large number of patients who were selling their medications.  I could never figure out how so many people were given such strong medication if they didn't even need it (unless they were getting huge amounts from other doctors or something).

The clinics around here in Minn. started doing that about 3 yrs ago.  I should say the bigger cities did it.
