Let me try to clear some things up for you if I can, and offer you some helpful tips.
For one, you don't "have to" snort or inject the subutex. Actually, doing so DECREASES its effectiveness. You will not experience worse w/ds if you take the medication the correct way. Actually, you will have more problems being maintained if you take it any way but how it's made to be taken. In addiction, the addict becomes just as addicted to the process of getting high (snoring, shooting up) as they do to the drug itself, so you definitely need to address those issues.
You've tried many times to get clean, yet keep relapsing. That means there's something you're not doing or addressing to help you STAY clean.
No matter HOW you actually detox off your drug of choice (cold turkey, taper, subutex), you still have to put a recovery plan in place for long term sobriety. Getting clean is actually the easy part, compared to staying clean.
You need to reassess what you've been doing and make the necessary changes, so that this will be your last time. For one, you need to eliminate all of your sources...dealers, doctors, however you get your drugs, you need to eliminate those avenues. If you don't do that, and you continue to have access, staying clean will be a big challenge.
You also need to increase your accountability. You can do this by telling some trusted people in your life that you're an addict trying to get clean. If you keep your secret, it's much easier to keep using. You would essentially be putting roadblocks up to relapse. Another way you can ensure accountability is by being compliant with your sub program. If you're just getting the sub, but not being honest about your snorting or shooting, then you're not really helping yourself. Same with if you plan on cheating on your tox screens. All of those things are in place to keep you accountable. Cheating the system only hurts yourself. Go into this with the intentions of doing whatever it takes to help yourself...that means working the program and all it entails. If it's a good program, it will require therapy and recommendations for AA/NA meetings and the like.
Lastly, you must have an aftercare plan in place. This involves things like addiction counseling, NA/AA meetings. Those kinds of things will give you the TOOLS you need to stay clean. If you're not planning on staying on the Sub forever, then you will eventually have to face coming off that as well, and if you haven't put the "work" in to really WANT sobriety, you will likely relapse again. If you're main focus is only the w/ds, then you're skipping a vital part of recovery. Of course the w/ds stink, but they don't last forever...addiction does.
It goes without saying (as you know) that snorting and injecting things that weren't designed for injection is dangerous. So, that's an even bigger reason to really give this your all. Every time you snort or inject, you're taking a gamble, and doing more damage to yourself.
