You have to do things slowly, you have to make a plan. The plan should be not of what you want to do, but who and how do you want to be. Once you figure that out make a schedule that you can keep. Nothing harsh or extreme that will make you exhausted just by thinking. Maybe cut white sugars first week and exercise 15 minutes. Than cut white flour next week and add 5 minutes to your exercising.
