Impaired immunity due to ENTRAK surgery?

Very complicated case, as I have impaired cilia in Maxillary, presumably from my last ENTRAK surgery 12/08. Had 2 surgeries prior to ENTRAK, but they were many years ago. Carrying staph, and  confirmed pseudomonas in sinus, that can't drain and refractive to antibiotics, even tho C&S showing suseptibility. Sinus rinses not option b/c goes into ear canal. Recovering from ear pressure started 6 weeks ago after rinse. Lost hearing from this episode. On steroids now, will see if pressure returns. audiology testing confirmed  pressure issues. Past rash and hives from quinolones. Toughed out 10 days of Levoquin,& ended last week with itching increasing.was on Benadryl Now ENT office wants oral Cipro 500mg bid x14 days. Took in January 09, but was desensitized for it. (Different ENT.) This office says if don't take Cipro, they will go to IV. I am 5 hours from this office,(Teaching hospital, ENT Fellow my doctor). I live very rural area. Quit previous ENT surgeon, he not admit that I still sick. On top of this Allergist says titers showing only 5/14 protective antibodies for strep pneumo after 2 pneumovax shots, but not Immunoglobulin deficient-just low range.I BZ mom, can't die yet, but afraid! pls help me. Is there an expert in US who deals with this? and how to raise immunity? Do I try to desensitize myself to Cipro? I have drug on hand and 4 EPI pens. I am also thinking I shouldn't get H1N1 vaccine if have active pseudomonas infection in sinuses. Feel pretty good, but know crash anyday.
