Hi, anal fissures are more common in the posterior aspect, but they can occur in other areas also. And yes herpes can present as a fissure. But with this there may be associated lesions in the skin nearby the anus, like the vulva or the thighs. But when in doubt this can be confirmed by blood tests for antibodies or PCR tests. Only a consultation with your primary care physician can help diagnose. Regards.
