heart attack

i woke up 3 am and was so full of flem (phlegm) i could hardly breathe is that a sign of heart attack

Breathlessness and increased respiratory secretions are not signs of a heart attack though these could indicate cardiovascular or respiratory issues. I would suggest getting this evaluated by a primary care physician initially and depending on the cause diagnosed/ suspected, it can be managed accordingly or specialist care may be sought.
