MEDICAL PROFESSIONAL
This is an Aural Hematoma caused by excessive shaking of the head.  Dogs will shake their heads excessively in trying to alleviate the pain or itching in one or both ears.  The ears are itchy or painful or both due to an ear infection most often secondary to allergies to foods or allergens in the environment or parasitic ear infestation.  Once the capillaries are broken in the pinna or flap of the ear and blood fills in the space it cannot get back out and will have to be drained by a vet.

If left untreated the hematoma will evolve into a fibrosed ear much like a cauliflower ear that boxers get from repeated hits in the ear.

Please take her to the vet when you can.  The vet can treat the painful, itchy ears and drain the hematoma.  Occasionally surgery is required to keep the ear hematoma free when draining is not enough.
