Caused by bad anxiety disorder?

I'm a 24 yr old female. I see a therapist and have been attending Day Hospital. I have not recieved a diagnosis, which I feel would help if had at least an idea. I suffer from deralization(which I was told is a form of dissociation),hallucinations(usually only happens when experiencing derealization),paranoia, anger(caused by the paranoia), I'm pretty anti-social and am very isolated, I self-mutilate(due to the deralization)I often have thoughts of suicide and have been experiencing quick mood changes lately, my thoughts often feel scrambled and is hard to communicate. I often draw blanks, have lack of thought and forget words. I sometimes feel on empathy at all, and sometimes get confused easily. I'm on Abilify, which has helped to a degree(mainly with the paranoia/anger, and my thought process. But my experiencing derealization has gotten worse. I do sometimes experience anxiety, and was wondering if I could have an anxiety disorder? I'm not looking to be diagnosed on here, just want an idea. Thanks.
