You will need a doctor's note. When you get your "packet" from the Court it will explain what you can do (reschedule, change location, etc) and what is needed to be excused (doctor's note). Good luck!

Call the people who summoned you for the answer to your question, because they get calls all day long from people who want out. I have bad hearing and called them about that, and they agreed that I don't have to go for my summons which happens to be next month.

I won’t need a doctors note? At the moment I don’t have insurance I recently quit my job so I’m a full time nanny while I attend collage! So I’m looking for good health insurance so I can get on medication for my anxiety.
There are two issues in your statement.  The first is whether medication is where you need to go if you haven't tried anything else first.  It sounds like your anxiety is pretty bad so medication does sound like something you might need, but I'd first get into therapy with a psychologist who specializes in anxiety treatment and see if the therapist thinks you can beat this without meds.  Meds are not benign.  When they're necessary, they're necessary, but if they're not, it's better to not take them, just as it's always better if you can heal an injury without surgery than it is to have surgery.  But if you need it, you need it.  The jury duty part is, yes, you can get out of jury duty due to anxiety.  Whether or not you need a doctor's note depends on your local jurisdiction, but you probably will.  I would caution, though, that although you mention you're suffering anxiety because of this, you don't say how badly this is affecting your life in the sense of avoiding things.  I know this is easy to say, but avoidance will make the problem harder to get over.  If you're someone who gets anticipatory anxiety but then does okay when you're actually doing the thing you're anxious about, think about going.  If you're someone who gets panic attacks and you've tried repeatedly to do things but doing them just doesn't help the next time you have to try something, that's a different story.  The more you avoid, the smaller your life will become and the worse your anxious thinking will get.  The most recommended type of therapy for anxiety, and even if you do go on medication you will probably be encouraged to do this, is a form called CBT, but no matter what type of therapy or medication you get, you will eventually be encouraged to try to do the things you've been avoiding.  I'm not saying this guarantees success, it doesn't, it didn't work for me, yet I still recommend highly you don't end up like me.  Because you're young you're far more resilient than you know, and now is the time to learn to relax yourself out of this type of thinking.  But the bottom line is, yes, anxiety diagnosed by a psychologist or physician of some type is an accepted reason to avoid jury duty.  Now, let me ask, as maybe we can help you out here, what is it about jury duty that triggers your anxiety?  It's actually kind of fun.  The first time I was called for it I battled my phobia, which was mainly of the fact the local courtroom was high up in the building and I had agoraphobia, including a fear of heights.  But I went, and my only regret was that I wasn't selected to serve on a panel -- once I got there I really wanted to be in a trial.  The second time was after a medication caused me to get infinitely worse, and that time I did opt out.  That's how I know it can be done, I've done it.  I would have preferred, however, to have done it.  Peace.
I have use to have a therapist for my anxiety that’s why i was in medication but after I graduate from high school i couldn’t see my therapist anymore and i didn’t take the pills anymore! But the older I got I noticed it got worse! And it’s bad to the point that I avoid and hate stores especially the mall! When I was in high school i had to eat in a teachers classroom because the cafeteria was so full! And now that i will be attending school next month I’m doing it online because of that reason! And either way I am a full
Time nanny so I don’t really have a choice. What’s makes me freak out is just thinking that I have to talk around people I don’t know! Seeing new faces!! Just the whole situation gets me so nervous and I start to over think everything and freak out.
I recently had experienced a anxiety attack which my anxiety has never gotten to that point! I would have little panic attacks in school but the anxiety attack I just experienced while I was babysitting a couple weeks ago was super bad! I had to call the ambulance and I was about to faint it was horrible.
You will have to call the jurisdiction that served you to find what they require to be excused.
I know the jury duty feels like the important thing here because it's imminent, but really, you need to get back into treatment for your anxiety or else it will keep feeding on itself.  So whatever you do about jury duty, please get back into treatment for the anxiety.
I called and I’m getting a doctors note to get out and I have health insurance now! So I have an appointment next week for my anxiety!
Thank you!
