I have that same problem. Went to the doctor recently and he said my muscles in my neck and shoulders were very very tense. I had been having an achey numb feeling in my left arm and the doctor said that those tense muscles would cause it.
Virtually everybody in the entire universe gets tense, sore and achy muscles for many, many reasons. And yes, many of us are not aware of how we end up with them. Poor posture, lifting heavy things incorrectly, physical activities, being hunched over our computers for hours on end, the stress of day to day life..........all of these things can cause the problems you mention.
Since you know your shoulders are "always tense," the first step might be to become mindful of this and remind yourself to relax your shoulders. You may have to tell yourself this a thousand times a day...........I know I do!
You can also learn some exercises you can do even at your desk at work. There are many excellent Yoga practices to help your neck and shoulders.
I seldom tell people to Google anything, but you can find some very simple exercises that will begin to help you almost immediately.
Since this is causing you enough anxiety to seek our help, you might benefit by seeing your doctor to rule out a medical cause for this. It IS possible you've pulled a muscle or perhaps injured your shoulder in some other way. I don't think it's anything serious, but I am NOT a doctor.
If and when you see your doctor about your sore shoulder, please be sure and discuss your anxiety. I would hate to see this turn into hypochondria for you.
