My paranoia and anxiety is driving me crazy

I had a pretty crap childhood and since then I've always been a very paranoid person. I have bad claustrophobia, I'm terrified of clowns and dolls, I'm scared of the dark and I have trouble sleeping because I always have this stupid irational idea that some kind of serial killer is looking at me through my window. I always think people are talking about me and lying to me.. or that theirs some kind of negative meaning behind things people say to me, even when they tell me they love me I feel like they seacretly can't stand me... and there's quite a lot more I could go on about to be honest.

I basically just want to know if all this paranoia and anxiety could be an actual phsycologicial issue, and weather I should get help with it or not?

i sometimes feel the same way too. with the help of anti-anxiety medications i learned to start trusting people and gain more confidence i still have paranoia that people do not have my best interest but now it is quieted by me not caring as much.
