Have your adrenals tested.

I agree totally with allmymarbles regarding having your adrenals checked, but don't let your doctor stop there. Have a very, very complete medical work up.
You had a panic attack 6-7 months ago which should not leave you feeling like you're constantly living in a dream world and now you have random panic attacks in which you fear having a heart attack, you have palpitations and some sort of "weird" and random twitches.
This sounds like pretty classic anxiety/panic to me, but you need to rule out any organic cause for these feelings.
If........after a thorough medical check up, your dx comes down to anxiety, you need to reconsider therapy and possibly a short course of medication.
Not knowing what medication you were on, what dose, or for how long, it's quite possible it was the totally wrong medication which has lead you to believe that ALL meds don't work. The same could be said about the counseling you received. It can take quite a few tries before we find the therapist that's right for us. And you'll know when you do almost immediately.
I realize I'm covering a lot of ground here, but I hate to see you make up your mind so vehementaly against possible options to help you. You say, "I can't work," "I can't live," and "I can't take this anymore!"
So don't then.
Get proactive in your own physical/mental health.
Knowing that you're doing something to help yourself is a very self-empowering feeling.
I wish you the best
Peace
