insanity?

i have heard that people who go insane dont feel any different but how can i feel this different and not be losing my mind?  i have been dealing with anxiety and low self esteem for quite sometime.  my internal medicine specialist says that i probably have sleep apnea being that my tongue and throat are enlarged and im obese. can this feeling of depression and constant anxiety and obsession with my sanity be linked to sleep apnea? i feel like my body is in auto pilot while my mind is somewhere else. as though the world is moving around me and im on the outside looking in. im on my 4th week of cymbalta 30mg and the previous three weeks i felt fine. its just now these amplified symptoms are surfacing. my dreams have been extremely vivid and cause me a great deal of agitation. my mind is almost constantly racing and distracting me from the things i need to do. can someone give me an idea of whats going on? is this just anxiety and depression or does it sound like bipolar disorder or some other mental disorder? thanks so much!
