klonopin

is it ok to take .5 mg of klonopin twice a day or should i stick to .5 once a day? which is more common. i tend to feel it "wear off" in the afternoon and feel as though i need another. my dog prescribed it not prn but once a day. but i am wanting to ask her to switch it to twice a day but am afraid she will think im just asking for more drugs.

Clonazepam (Klonopin, Rivotril) should be dosed at a minimum of twice daily, as this will utilize the accumulation of the drug. The only exception to this rule is if it is being used on an as-needed basis (once or twice/week). When dosed twice daily, the drug accumulates to 1.5 times that of the steady-state plasma level within two weeks. As an example, those who are taking 0.5 milligrams twice daily have the plasma equivalent of 1.5 milligrams. This accumulation umbrella maintains the plasma level over a duration of 24 hours, and hence, there is no "roller coaster" effect when dosed in this fashion.

Onset of action is 45 minutes - 1 hour, the peak occurs after two hours, and the duration of action is 12-18 hours, or 8 hours for infrequent, as-needed use.

Dosed three times daily, the drug accumulates to three times that of the steady-state plasma level over a span of one month. This may or may not be desirable, and depends on the individual. In many, it will cause excessive accumulation and motor impairment when taken more than twice daily over the long-term.

I will also note that Clonazepam should NEVER be dosed more than three times daily, as the drug will accumulate in your liver and potentially hurt you - particularly at dosages in excess of 2 milligrams daily. Many physicians are not aware of this.
