Arm and finger pain

For the past several months my right arm feels like I have a tourniquet around it, also my elbow is very painful as well as my right hand  and fingers are swollen and painful. Could this be arthritis? But what about the forearm feeling like pressure around it and my elbow? Can someone please tell me what this could be? Could I have a heart problem or poor blood circulation?  I do not have health insurance nor do I have a doctor to go see.
Years ago I was diagnosed with possible lupus or  a autoamuine  but the doctor could not be certain could this all be related? I’m a 47 year old woman. And I’m very active, I do however smoke.

Hello.  I am certain that the symptoms you are experiencing are scary.  Based on the information you provided, there are many possibilities from the mundane to the more serious.  Your hand being swollen and painful could be related to arthritis but there are other ailments that can cause that symptom.  
I know you wrote you do not have insurance, but is there a free clinic in your area?  It would be best if you had a doctor look at this.  Barring that, have you tried putting ice on your elbow?  Have you elevated your hand to reduce the swelling?  How long has this been going on?  Does anything you do make it better or worse?  Have you tried taking aspirin for it?
