HIP MRI Scheduled in Two days.

Hi , I have not been on the boards in a long time, I have long standing chronic back pain from an on the job injury, My back initally showed on MRI a herniated disk at L-4l-5, but with DDD, I now have basically moderate degenarative changes L-1 -S-1, disk space narrowing,canal narrowing , osteophytes, etc. This is all from the last MRI in 2008, but this is not what my question is about, but just some background info. I hurt my back in 1999 and was laid off work in 2000 and have not worked since. I am now 58 years old and on Social SEcurity and SSI..I have also been on Methadone for pain since 2004. I also have facet pain and Sacroliac pain. For the last nine months or so, my hips esp my left hip has been causing a lot of pain and I at long last got a X-ray a couple of weeks ago and am scheduled now for MRI on May 1st. Can anyone explain what this may mean or any other possibilites please.?

findings: There is a faint subcortical linear sclerotic line with some subcortical lucency noted in the superolateral aspect of the left femoral head. There is mild articular cartilage loss in the superior aspect of both joints but is not out of proportion to what is normally seen in a patient this age. There is a small osteophyte noted coming from the femoral neck on the left. Bowel gas pattern within range of normal.

Impression: Mild Osteoarthritic change is noted in the left hip but the dominat finding is a subcortical lucency with a scerlotic line in the periphery. This can sometimes be seen with microtrauma and avascular necrosis. There is no articular collaspe at this time. NonContrast MRI of left hip is suggested.

Can anyone shed more light on possible meaning or other causes of this finding. Thank you as the MRI is only two days away. This hip is very painful esp when sleeping and stiff and painful in morning and the Methadone does nothing for the pain. I have to sleep on my side as my back hurts too much to sleep on my back, but lately my hips hurt worse than the back!          thanks            fluffypurrcat

Hello, thank you very much for taking the time to answer my questions and concerns. I had my MRI which took slightly longer than one hour. I was told the dr. would get the results tommorow, (2 days for results) so I presume the dr. will call me tommrow if anything shows up, or by the next day , Thursday, only because of the time needed to go over reults when busy with patients, etc.

       My question is if looking at soft structures, such as the tissue, what could they be looking for? I know I have some Arthritis, that is to be expected. What could be other possible causes for the  continuing left sided hip pain that has been increasing over last year ? The Methadone I take for my back pain does not help this hip pain at all!I do have a diagnostic back injection scheduled this next week for the L-4-L-5 and S-1 area, what I think is allso called a diagnostic l4-l5-s-1 medial branch block. to see if this triggers pain. My question : Is the dr. trying to mimic the pain I expereince , hoping that an injection in this area ( if one of the correct areas) will show the same pain I have when doing things such as bending etc.? If the injections shows that one of these areas, ( facet I think?) is the originator, then a Neurotomy can be done? I do know I have not had relief with epidurals and a SI inection in the past. If my hip is causing a lot of this pain, then will the test still be postive or only if it is the facet joint or area of the back, not the hip itself? Thanks,             fluffypurrcat

Hi there!

Sclerosis refers to fibrosis/ calcification of a region of done seen in degenerative processes (minor trauma/ reduced blood supply) with loss of cartilage that covers the bone in the joint. Cortex is the outer region of the bone. Osteophytes are small bone outgrowths, commonly seen in degeneration. Since these are faint/ mild changes these are unlikely to be grave. Detailed imaging may be suggested for a clearer view and to look at the soft tissue structures.
