Musician needs help with trumpet finger pain and disconfiguration

dominant index finger joint closest to the fingertip severe pain, nodule, swelling and disconfiguring to pointing outwards towards left hand.

Well, without a clinical evaluation it would be difficult to determine the cause of his symptoms. Possibilities that may need to be considered include inflammations, degeneration, arthritis, growths/ masses, fractures, dislocations, causes related to the tendons/ ligaments etc. I would suggest getting this evaluated by a primary care physician or a rheumatologist for an accurate diagnosis and appropriate management.
