This is an answer to people with hand and arm/shoulder pain.  I had a cervical neck fusion in August 2008 and after the surgery I continue to have pain in the area that troubled me before the surgery.  However, I am working desperately to stop the pain in my forearms with velcro straps that I pull tight above my elbows and this has really helped and stopped the continuing muscle spasms.  Also I have been doing a beginning simple yoga tape to stretch out my arms.  I also have been using the putty clay kneading my hand and fingers while watching TV at night.  The pain is getting less but still not gone but everything that you I have done to help myself is making an improvement in the quality of my life.  Pills are not the answer for me and this is a lot more reasonable and you just have to take the time so you can feel better each and everyday.

My sister had similar severe hand, wrist and up to the shoulder pain as well. The pain was so bad she actually ended up sleeping in a recliner so she cou8ld hold her arms in just the right position. She also has RA like me and just attributed it to that. Then she was recommendtd to get checked for Carpal Tunnel and low and behold that is what she had all that time (still has RA). She found a surgeon who only does carpal tunnel surgery and had one wrist done and was in instant relief. Three weeks later did the other wrist and she has been in heaven since without all that pain.

Get checked for carpal tunnel, worth a try.

PS - the surgery was out patient and recoup very quick.

Excellent incite from New Leaf.  I agree that you should investigate further and Carpal is a likely culprit.
