What does this mean?

What could cause an elevated ESR and CPK with muscle aches and burning sensation under the skin.

The elevated ESR indicates inflammation in your body and the joint pain, muscle aches and burning sensations can be indicative of several different forms of autoimmune disorders including arthritis.  Are you seeing a rheumatologist?  They are specialists in autoimmune diseases and are best equipped to diagnose what is happening with you.  Have you had other tests?  Is there a family history of autoimmune illness in your family?
