migratory joint pain in my child

My 9 year old daughter has been having migratory joint pain off and on since May of 08. At first blush Dr's said it looked exactly like Rhuematic fever however blood tests are neg for this. They have since tested for Rhuematoid Arthritis, ANA negative, Lhyme disease titer negative, her sed rate was 35 at highest and last test was 8. White blood count normal and all metabolic rates normal. The joint becomes hot and swollen and painful. no rash or fever is ever present, discomfort will normally last 3-4 days and then go away. We are seeing our pediatrition and a pediatric Rhuematologist. Every one is at a loss as to what to test for next and we are treating her with naproxen 2 x daily and tylenol for pain as needed. Theory at this point is to keep on naproxen for up to 6 months and watchfull waiting. As a perhaps side note she was diagnosed with ADHD a year ago, she is not on any medications for this. She also has been having night terrors off and on for the past year. I am wondering what other test's would be rational and if there is another type of specialist that would be wise to see at this point. Thanks for any help or direction you may be able to give. LWH

The worst feeling is when the doctors are at a loss.  You wrote that they have run blood work, what other tests have they run?  Red swollen joints are certainly indicative that something is going on.  Sometimes these things do not show up in blood work.
If your doctors are at a loss and not sure what the next step should be and they are not grabbing peers for consults or cracking books looking for answers, it is time to get another opinion.  Another ped rheumy would be good but you might also want to have her checked by other specialists such as an infectious disease doctor.  (Not that I think she has an infectious disease, but that is all that is coming to mind at the moment and it is certainly worth checking since nothing else seems to be panning out.)
Regarding the ADHD diagnosis.  Perhaps she is not ADHD but has symptoms of it in relation to the joint problems.  If her joints are hot, swollen and painful I am sure that it is affecting her sleep and that could be related to both the ADHD and the night terrors.
Most of all, I want to encourage you to not give up.  Keep pushing the doctors for next steps and keep going to as many doctors as needed until you find one who can either help your daughter or can gather the resources to do so.  It is a frustrating process as a patient and I can only imagine how helpless you feel as a parent.  Please let us know how you are both doing and we will help in any way we can.
