FDA Warnings

How do pharmacists learn of FDA warnings such as the one issued on Nov. 17, 2009 regarding the incompability of PLAVIX with PPI's, specifically PRILOSEC?

Should the person responsible for over-seeing the central pharmacy or dispensary for a prison system be aware of such warnings?  If so, would they have an abligation, either legal or ethical, to take action to protect the patient, namely prison inmates, from such dangerous incompatibilities?

Pharmacists usually learn of FDA warnings through the mail or through emails. The pharmacist should be responsible for warning the patients of any dangerous incompatibilities of drugs and the doctors should also be notified.
