How low of morp SR before I can start Suboxone?

""How low do I have to be on the morp to start Suboxone and what dose of Suboxone would you think is suffient?""

  I have legit chronic pain in many places and need something.  My PCP is a NP at Kaiser who is a joke.  Almost killed me and ordered wrong medication and is not a smart fella!!  

Any other med you would suggest besides Suboxone?

Current Medications
:  
was at 600mg SR morp for 2 years and other narcotics for 4 years BEFORE that.  Now I am decreasing and down to 180mg of SR morp.  

MORPHINE (MS CONTIN/ORAMORPH SR) 30 MG ORAL SR TAB
Instructions: Take four tablets (120mg) by mouth every 8 hours around the clock for chronic pain
. Was, now down to 180mg.

ALPRAZOLAM (XANAX) 1 MG ORAL TAB

CLONIDINE (CATAPRES) 0.2 MG ORAL TAB
Instructions: Take one tablet (0.2 mg) by mouth twice a day

ONDANSETRON (ZOFRAN ODT) 4 MG ORAL RAP DIS TAB
Instructions: Take one tablet by mouth twice a day if needed for nausea or vomiting

NITROGLYCERIN 0.4 MG SL SUBL TAB
Instructions: Dissolve one tablet under the tongue every 5 minutes for 3 doses if needed for chest pain

LISINOPRIL-HYDROCHLOROTHIAZIDE (PRINZIDE/ZESTORETIC) 20-25 MG ORAL TAB
Instructions: Take two tablets by mouth every morning for hypertension

You can start the suboxone now. The usual first-day dose is 4mg twice a day. You will need to be closely monitored by your doctor.
