You should definitely have your ear reexamined, either by your doctor, if you still trust his diagnostic skills, or preferably examined by an ENT specialist.  This reexamination should be performed without delay for, if there is pus or blood behind the ear drum, it may have to be drained.  If it remains undrained and/or unresponsive to the amoxicillin there is the possibility that it could result in some permanent loss of hearing.

You could have accidently traumatized your ear with the Q-tip but it is more likely that the blood is due to your ear infection.  Unless your ear canal is blocked, the loss of hearing is an indication of fluid and/or pus or even blood in your middle ear, behind your ear drum.  If pus, it would be important to make sure that it has been caused by a bacterium that is sensitive to the antibiotic.  It would be wise to have this taken care of, without any further delay.

Good luck.
