Diagnostic Checklist Form E-2 and Research Questionnaire Form E-3
A high-priority goal of the Autism Research Institute (ARI) is the development of improved methods of diagnosing children with severe behavior disorders, since most medical research is highly dependent upon accurate diagnostic methods. Our Diagnostic Checklist, Form E-2, has been designed for this purpose. In addition, the Form E-2 checklist asks parents to rate the effectiveness of various interventions they have tried on their son/daughter. Information on the effectiveness of interventions is compiled on a regular basis and is shared with families and professionals throughout the world.

Form E-2 is available on request in English, French, Spanish, Portuguese, German, Italian, Hebrew, Japanese, Turkish and Serbo-Croation. Our files contain information on over 35,000 children from more than 60 countries. Our service to researchers depends in part on our having a comprehensive bank of up-to-date information about children with these severe disorders. Your help in completing the E-2 checklist and submitting them to ARI will be greatly appreciated.

Diagnosis. The responses on Form E-2 are entered into a computer which calculates a diagnostic score. The score along with its interpretation is sent to the person or agency submitting the checklist. In addition, a file number is assigned to each child for data recording and analysis, and to ensure confidentiality. There is no charge for this service.  It usually takes a week to get your report back.

Thank you for sharing the link.  I will have to fill this out for my daughter.

Personally, I dislike the true false questions... some of the questions I'd have to answer with True in some cases and false in others... and some of the answers would have changed from last year to this year... and some questions are true with strangers and not mommy and daddy.  Is this supposed to be a diagnosis?  Or a confirmation or a diagnosis?  Or just a way to collect information and store statistics?  It is good that they are doing something for parents and people.  Actually this is similar to the questionarre form thing my daughter's developmental pediatrician originally gave us.  I filled it out and I remember the doctor getting mad at me because I couldn't answer simple yes/no questions.  He correctly figured out that I have mild OCD and ADD even though I didn't list it on my health issues list (which I didn't really think OCD and ADD were health issues since they don't make you sick or ill but I guess they are mental health issues or something).  ha ha ha... took him less than 30 minutes to figure out that when my doctors growing up didn't diagnose me until college.  

Well, I'm digressing... anyways, maybe I'm the only one who hates yes no or true and false questionarres because someone who might have a mild form of some sort of autistic spectrum disorder would be told they are fine and maybe someone who is not on the spectrum might be diagnosed with being on it.  

So I guess my point is... questionnaires are good and helpful, but you shouldn't use them to diagnose a condition -- though I guess with the lousy regular pediatrician's who want to say everything is okay with your kid, maybe this is the only way parents are going to realize they need to take their child in to see a specialist who is trained to diagnose these types of things.  Oh and if you happen to be in a cruddy school district that doesn't have teachers or staff to recognize issues and say things need to be addressed and give your child help.  I am not sure if school systems get more money if they can label children as autistic or if they don't have enough in their budget to give therapy and help to children who have autism so they don't want to have kids in that category or if it is a combination of both depending on which school district you live in.  

Uh, I didn't mean to like trash the questionnaire form... i do think it's a great thing, and I do intend to fill it out and mail it in.

Oh, I just reread the post... it looks like the reason for the questionarre thing was listed... but my reading comprehension has always been kind of poor... I focus in on a couple things and get sometimes get sidetracked from other things that are written... Um, anyways, kudos for posting the link...  sorry for my ranting/raving about true/false/yes/no things

Oh, another comment... the things i was being picky on were not the majority of the questionnairre... just certain parts of it.  And the true false thing was E-2, Part 2 that I didn't like...  Okay, so now everybody thinks I'm crazy to keep posting...
