regression

My 8 year old is regressing, he holds his poop, and then poops on self, he has aspergers, and has done this since school started, he won't tell his teacher, and I started to repotty train him at school and home. Why could this be happening now.  He learned to potty at 4 1/2 years old, and would  hide to go poop on diaper. I've and have hospitalized him for poop getting stuck and we had several emerency room visits.  They call it encopresis, and he still continues to hold his poop.  He takes miralax and is on a high fiber diet.  My son could not speak until he was 4 and lost his mind when he was 6, he was suicidal.  I have tried the reward system and it works sometimes.  He gets real hyper and then real sad a second later like somebody died.

For the poop getting stuck:  Magnesium citrate works to help break really hard #2 up (if your child holds it in for too long) combined with doing miralax (not at the same time, but I usually do the miralax and then do one day with the magnesium citrate without the miralax if the increased dose of miralax didn't get it out.  You can get magnesium citrate over the counter.  Ask the pediatrician or pharmacist before using it, though.  I got permission to do so for my daughter, but I had to inquire with the doctor... the doctor seemed to prefer the enema route, but the magnesium citrate is a lot less horrifying to your child.
