Low Potassium Levels and Autoimmune disease

I was in the hospital this weekend with VERY low potassium levels, as well as other CMP levels.  Just wondering if anyone else has struggled keeping their numbers up.  I do take diuretics for heart failure, but even without those my levels are under normal.  My vitamin D was also in the basement....Just wondering.

My Vit D, Calcium, Iron, are always low, lower than low! If I do not take supplements I would literally die.
  My potassium is pretty good though!
I have Hashimoto's Disease. Which is an auto-immune disease. Many thyroid patients have low Vit D too. There seems to be a correlation between low Vit D and auto-immune diseases, but why has not been established.
