It could be a lymph nodeLymph node biopsy Swollen glands Swollen lymph nodes in the groin Swollen lymph nodes under arm. You have lymph nodes behind both ears. If you have allergy problems or get sinus problems, or anything like an upper respiratory illness those nodes, or just one, can swell up and become really tender. I have the same thing. I have bad allergy flare ups and it feels like a knot behind my ear. If you mess with it then it becomes more tender.
