The report suggests that you have scar tissue pressing into the opening through which a nerve root comes out off. This can cause back pain and leg pain.
You may benefit from a nerve root block and physio.
If all else fails repeat microsurgery with either fusion or dynamic implants may be required. There are anti-scar gels that can be placed during the next surgery to reduce the chance of recurrent scar.
You may wish to consult with a neurosurgeon.
Thank you for your interpretation, i will keep that in mind as a decide what my next step will be. My ortho showed me on the cd/film that my disc is herniated and he showed me that my nerve is swolen, it had been compressed for a several months before i had the discectomy. he now recommends a spinal fusion, he didn't say much about the scar tissue around the neuro foramen, and i know that is an issue that adds to my pain as well as the herniation that may be compressing the nerve. In addition to these problems i am having active sarcoidosis. it is noted in the file with the ortho, but i think he has been looking over it, or frankly he conveniently has forgotten.whatever the case may be, it is to be taken into consideration before a spinal fusion is to be done.
