Back shoulder pain- Left side- after work

Hi
Lately I have started getting pains in my left upper back, near shoulder which continues to the left hand. After a while the entire left hand start paining. This is started to be frequent for the last 1 year and happens only when I do work.
Also i feel tired very quickly by doing bit of work.
I am a left hander and recently married.
Kindly advice me, as what could be the possible reason for this and what is the remedy. I am a bit worried.
