Fall on back on icy steps

I stepped out of my back door Dec. 23 in the evening onto icy steps. I slipped and fell hitting the center of my back on one step and the back of my head on the step above it. Pain was severe the next day with nausea from the pain and has gotten slightly better over the past couple of days. My back is swollen and very tender, but no bruises (yet). It is difficult to sit down and then stand back up. I had a large "goose egg" on the back of my head which is slowly going down. About how long should this pain last for before I should see a dotcor about it?
