How would I know when I should see an orthopedist for a pain between my shoulder ...

How would I know when I should see an orthopedist for a pain between my shoulder blades that started over two months ago?

Hello,
I am hoping you may be able to help me.  I have had pain in the center of my back between my shoulder blades that is not letting up.  It doesn't hurt to do anything with it, however I have pain there 24/7 and I feel it when I am doing nothing, just sitting and when I try to stretch it feels like my muscles are tied so tight that I can't stretch them out.  This pain started two months ago, it came on without warning and without injury.  I need to mention that I had severe neck pain in March 09 and it was so bad i called my ortho,  they did an mri and while i was sure it was a herniation of the cervical spine, it unfortunately turned out to be a demylenating lesion at the c4 level.  nothing happened to me then for that to occur, however i had major surgery (fusion of the l4-s1 with hardware) and my body went through a lot during the surgery and post op recovery.  the trauma from the surgery seems to have caused my body to turn on itself, but this middle back pain is mysterious to me.  Again, I did nothing to bring this on and at times it has been absolutely unbearable, and at times it has just been annoying.  I had some problems with sensitivity to my skin from the middle of my back reaching around to my sternum.  If you drew a line from the middle of my back to the front of my chest, you would have been able to draw a straight line from back to front, i couldn't even wear a bra or have anything touching my skin for a couple of weeks.  can this happen from a herniation or a nerve compression.  should i contact my ortho to discuss, or should i discuss with my neuro who is still reluctant to dx me with ms, waiting for another lesion to be identified, separated by time and space.  Would welcome any thoughts you may have that will help me know which direction I should go in (neuro vs ortho). If it was herniated in that area of the thoracic spine, would i have radiculopathy and if so what and where might that be.
