Surgery for t6-7 herniation with cord compression

Is it the last resort " surgery " when the diagnosis is herniated disk t6-7 with moderate cord compression ?

most herniations do not require surgery, and also are not the cause of pain. most people over the age of 30 have more than a few herniations. pain is usually muscular in origin

At T6-7, there is moderate size midline disc herniation with moderate cord
compression. The disc appears somewhat hypointense on the T2-weighted images
would this require surgery ? if leg pain and muscle spasmatice episodes with weakness is occurring ?

i would have nerve testing done by a neurologist----- T spine surgeries are quite difficult i believe. difficult to access.
