What do I do?

MRI Impression: L4-5 there is an annular bulge with a tiny central disc herniation demonstrating mild superior migration and a broad based right posterolateral disc protrusion occupying the inferior portion of the right L4 neural canal. At L5-S1 there is a left hemilaminectomy defect. There is a small right paramedian posterior disc herniation at this level which is resulting in some degree of mass effect upon the exiting right S1 nerve root within the region of the lateral recess.
1) Have had 3 epidural injections
2) 1 SI joint injection
3) Months of physical therapy
4) NOT recommending surgery and not sure why
5)Still have horrible numbness/tingling in both feet
6) right glute pain
7)muscle spasms
