What is this pain in my shoulder blade & spinal area?

I have had pain in my left shoulder blade for over  year now. I started noticing it right after my final collegiate soccer season. I have always had lower back pain due to my minor scoliosis and right hip being turned wrong and playing sports, but this pain is different. It is sharp and stabbing down my spine and under my shoulder blade. It hurts mostly when I lay down and get back up, sneeze, do push up or any exercises like that, and is constantly aching. I am living off of ibuprofen and I don't want to because I know how bad that is for me. Physical therapy didn't work and neither did a chiropractor. No one really knows what is wrong. Any idea of what it could be or what my next step to healing would be?
