back pain

Hi, Does any one know what this means? AT L5/S1, there is broad central disc bulging with annular tear abutting the thecal sac and in close proximity to the S1 nerve root origins? keep in mind this is on an mri results and also shows at L5/S1 degenerative change at L4/5-L3/4-L2/3-T12/L1 mild disc bulging in this and T10/11 mild disc bulging and mild anterior osteophyte. if anyone could please find out what all this means and what can be done and if I will have this the rest of my life I have tried slow release pain medication actually tried 8 diff medications now getting trigger point injections but not helping yet. I would really appreciate any help on this.
