disc desication at L5-S1

On my latest MRI the impression said:  

i also have fluid in the facet joints at L5-S1.

1. Central and left paracentral disc herniation at L5-S1 effacing the epidural fat and impinging on the thecal sac with findings consistendt with an annular tear within it. The herniated disc is smaller than on the exam of 2-15-07 but persists and the annular tear is more defined on the present study.

2. Capacious lumabar canal. Facet joint tropism at L3-4, L4-5, and L5-S1. no abnormality seen in the intervertable discs at L1 through 5.


All i know is i am in a lot of pain and i dont understand what all this mumbo jumbo means could someone plz tell me what it means and if surgery is the only treatment option i have. I am only a 23 yr old female and i want to try pain management instead of surgery but i dont know how to tell my doctor can someone plz help me and tell me what to do?

I am 57 years old and have suffered from back (mild) pain for many years.
And I have have had muerous MRI's and CT scans.
And you are right this is mumbo jumbo.
The MRIs are gnerally read by a radiologist.

I live in NYC and a family fiend- an orthopedist suggested I see his friend who ran the back/spinal suregery center at Joint Disease Hospital in NYC, He told me to bring my last two MRI films with me-he did not want the report. Why? Because he could read them and understand these much better becasue of his experience. I did and asked him what the report (I did bring it) means- and he explained all to me. No surgery.

Find a reputable spinal/back surgeon , bring your films, and "demand" he explain everything to you.
And this before pain management, becasue they will suggest PT and give you narcotics.

That is my opionion and advice

I agree start with a reputable orthopaedic spine specialist.  You are very young but pain can wear you out physicially and mentally.

Hopefully he will recommend some physicial therapy and perhapys epidural injections.  As long as you do not have nerve impingment which not all tests can reveal (unforunately) you may be able to get through this with occasional flareups.

A second opinion never commits you to anything.  Don't be afraid to ask questions.  Being an informed patient is important.
