have a stabbing like pain in L shoulder that extends all the way across

I am a 26 yr old female that has had an acute on set of this. A few nights ago I was just laying there and all of a sudden had a sharp pain in my shoulder blade that cut off my breath. It has come and gone within the last 3 days. It has gotten to the point that I cannot look down without a straineous pulling sensation. It hurts to the point that I cannot take a deep breath with out pain. Now I have a burning sensation in the upper part of my back that is not reflux. Should I go to the er and see what is wrong since I cannot breathe with out a stabbing pain now in my breast? Please let me know what I should do...
