Hi there!

A stroke at basal ganglias unlikely to be related to localized pain in the shoulder, though a person could occasionally have altered pain perception. Shoulder pain may be caused by capsulitis, bursitis, tendonitis, injuries/ tears, joint subluxation/ dislocation, arthritis, periostitis, muscle/ ligament/ tendon tears/ injuries, fractures, growths/ masses, ischemia, spinal issues etc. Well, without a detailed description or a clinical evaluation it would be difficult to come to a conclusive diagnosis. I would suggest consulting an orthopedician for a detailed evaluation and appropriate management.
Hope this helps.

Take care!
